I'm asking if it's possible design a route on the smartphone with tread app having bluetooth but not the internet connection?

Such condition happens when I'm in (not) rare situations and there's no cell coverage.

I suppose simply by synchronizing the only waypoints via bluetooth (not internet) and asking the XT2 to do the routing.

Not that I am aware of unless you have a routing program that isn't web based.
If you have plenty of Waypoints saved, then building a route using those is quite straight forward.
Avoid changing the Via points to shaping points after you have created the route - the Zumo will probably move them to a faster road and rename them.
